Continuum Rehabilitation is a rapidly growing therapy provider in search of talented and motivated Physical Therapists to address our significant caseload in the Philadelphia region with an outstanding opportunity for multiple therapists in Chester County.      We have recently been awarded the contract to provide therapy services for an upscale 800 + member Continuing Retirement Residential Community which are now called Life Plan Communities.

This pristine community is situated in one of the most appealing areas of the country, where you’ll be continually inspired by the beauty of your surroundings. Located in Honey Brook, Pennsylvania in Chester County, the community is bordered by miles of pastureland and rolling hills populated by Amish family farms.  This life plan communities location also provides easy highway access to Philadelphia, Lancaster, Reading, and other metropolitan areas that are about an hour’s drive away. The campus features lovely private homes, cottages and apartment residences, with distinctive amenities. This area is just outside the western suburbs of Philadelphia.

Full and part-time candidates will be considered to help with the increasing home care client demand. Our therapists receive competitive per visit rates with great flexibility and independence in their treatment scheduling.


We started our Philadelphia operations in late 2015 and are expanding rapidly. Continuum has been actively providing rehab services in Pittsburgh for the past fifteen years and for the past five years in Rhode Island and Connecticut.

  • Strong communication skills, flexibility, and professionalism

  • Successful completion of a program approved by the National Board for Certification in Physical Therapy
  • Current PT licensure issued by the state of PA and a current CPR certificate
  • Proficient in technological devices/systems such as tablets, computers, laptops, e-mail, text messaging, and electronic documentation (We provide significant training as needed)


Salary & Benefits based on schedule Fulltime or Part-time:


  • Fulltime will be considered when a therapist has 5 home visits a day/ 100 month
  • Compensation is based on home visits, you control your earnings and fulltime therapists receive our benefit package
  • We have therapists doing up to 150 visits month / each therapist is compensated at a significantly higher reimbursement for each visit over the 100 a month


Therapists are eligible for fulltime benefits at 100 or more visits per month:

Student Loan Repayment Program: Continuum offers a unique Student Loan Contribution Program, administered by


Two Levels of Benefit: There are two levels to the benefit based on number of visits per month, 100 Visits/Month and 150 Visits/ Month. The amount of principal that Continuum will pay down on your behalf increases each year and by the number of visits.


The contribution ranges from $150 a month to $550 a month by year five. We are committed to having our therapist enjoy a career with Continuum.
